Hope Over Heroin, most recognized for its faith-based outdoor rallies, recently launched its new Partners in Recovery Continuum of Care Program. This program connects those who reach out for help to a peer recovery navigator and/or faith mentor to guide them through a six-phase path to successful recovery. Hope Over Heroin’s team assesses, modifies, and tracks the individual through each phase of treatment and recovery, allowing them to provide accurate outcome reporting not limited by individual facility reporting requirements. The goal is to return people to productive functioning in the family, workplace, community, and church.
